For patients who are due an annual asthma review.

Please would you answer the questions on the form below and submit it to us.

If your symptoms are deteriorating or you have any concerns, please make an appointment to the respiratory nurse or a doctor as well.

Please note if you have used more than two reliever inhalers you need to book an appointment with a clinician for review of your asthma and medication. It would help us to gain a better understanding of your symptoms if you could complete the Asthma Control Test and bring it along with you, it can be found at www.asthmacontroltest.com/en-gb/welcome

A good inhaler technique can make a difference to how well you manage your asthma. It could also mean you’ll be able to manage your asthma symptoms without needing to be prescribed higher doses.
